Most of those countries have a life expectancy of less than 60 years old, people don't get old enough to be at-risk.

Funnily enough, their obesity rates are pretty bad

Even in Swaziland, heart attacks are the leading cause of death, right alongside HIV, malaria, diarrhoeal diseases etc

This is true, and was my argument as well, but even so, they still have more people over 60 (in the entire continent) than say the US or the UK. I think it's more likely they don't test everyone who coughs one time.
