It wasn't a fluke, it was a major fuck up. They emptied the powder from casings and reseated the bullets, to make rounds that would look real in the cylinder when viewing the gun from the front. They left the primers in the casings though, so when someone pulled the trigger it created a squib, lodging the bullet in the barrel. The gun was then loaded with blanks for the scene where Brandon Lee was shot, and the force of the blank load propelled the squib out of the barrel with lethal results.

After Brandon's death Hollywood went full-bore on weapons safety and it's standard to always run a rod down the barrel of every firearm between loadings as well as before it's ever handed to an actor or extra to check for obstructions or squibs. And no more live primers in dummy rounds either (what led to Brandon's death). Dummy rounds have punched primers. Blanks are a different matter. After The Crow shooting all prop guns were designed in such a way that real bullets can never be put in them and fired because they won't fit in the chamber. Non-firing prop guns (i.e. "post-production special effect firearms") don't even have firing pins in them and are configured so that it's impossible to put a firing pin in them.
If we're lucky we'll never see Alec Baldwin in a new film or on TV again.

From that one picture they took of the cast and crew as well as the location, this looked like it was already operating on a shoestring budget so they probably hired some cheap, down-n-out, back-alley L.A. prop company to handle everything including weapons.
On the big action films with tons of gun play a production company will often just hire a completely separate specialty prop weapons house to provide not only the weapons but the licensed and bonded personnel to handle them and train the actors/extras using them in the film.
They'll then hire out a separate prop company for everything non-weapon related.
Bottom line, it sounds like they might have been cutting corners on production and it wound up killing someone.

It happens. Blanks are quite dangerous at 3 feet. Read up on how Bruce Lee's son died making the Crow. Brandon died from a prop gun round in a bizarre fluke.
It wasn't a fluke, it was a major fuck up. They emptied the powder from casings and reseated the bullets, to make rounds that would look real in the cylinder when viewing the gun from the front. They left the primers in the casings though, so when someone pulled the trigger it created a squib, lodging the bullet in the barrel. The gun was then loaded with blanks for the scene where Brandon Lee was shot, and the force of the blank load propelled the squib out of the barrel with lethal results.

A blank round in a gun uses real combustable materials. The muzzleflash and sound you are seeing is part of a controlled explosion.
"blank" rounds aren't something you can just run around firing at people - the muzzle explosion displaces a SIGNIFICANT amount of pressurized air at a very strong velocity. So what you have is very hot air moving very fast.
So basically - prop guns that have blank rounds, are to be treated AT ALL TIMES as a loaded firearm. This means no shooting them around willy nilly.
Likely a few things could have happened here:
in between takes, the cinematographer was choreographic a gun play scene and was in the line of fire in close proximity to the barrel, the report said she got shot in the chest. She could have died from internal bleeding as well as trauma.
For whatever dumbass reason there was a live round inside the firearm. In this case - the prop guy will probably go to jail for manslaughter.
Alec baldwin was fucking around on set and he didnt know a blank round was loaded and the prop guy was too much of a pussy to tell him not to fuck around with the firearm. Maybe they were all fucking around on set and baldwin impulsively shot her as a joke.
There was a faulty firing pin inside the weapon, or the weapon was discharged accidentally due to a collision with the cineamtographer.
Im betting it was likely either 1 or 4. As those are the most likely scenarios where the Director of Photography would be in that close proximity to the weapon. Professionals will confiscate the firearms in-between takes in order to reload them.
If the cinematographer was wearing a camera rig and was choreographing a handheld camera movement during a take - baldwin would be holding the weapon, otherwise a standin would likely be there.
But 3 is also a possibility because baldwin was a producer on the film which would give him an added fuck you aura to ward off any overzealous prop guys.
Even if there was a live round in the weapon - he would not have intentionally shot the DP, unless he was being an absolute moron with the weapon, and hes had enough training with prop guns firing blanks to know not to do that.
