There is nothing stopping the one on the far right have as clear skin as the one in the middle. The one in the middle is what women with good healthy skin look like. Skin quality and skin condition is a sign of health and nutrition, and is intimately connected to condition of internal organs. We are naturally more attracted to clear and toned skin because it is nature's sign of good health.
The middle image can also be achieved through means of facials and microderm, not just makeup.

Does it need to be? I'd take right any day of the week but that's just me. Also, is middle truly "clear skin" or just makeup? Lighting too, perhaps?
