We could communicate with anyone we wished to, in fairly short order, or as short in order as required using established companies that specialized in getting messages across the world to any destination.

Also I'm curious how we would even know who to retaliate against since it's possible the origin of an attack might not be immediately obvious. Would we just attack the entire world? Let's say a first strike does serious damage but still lots of people are left alive and many cities undamaged. Do we retaliate if that guarantees the rest of the cities will be nuked? Or do we cut our losses? Who do we retaliate against? What will the elites in power do who have their own selves to think about? If they're a survivor of a first strike why provoke a second?

The way this works is that an airburst vaporizes the bomb components and they just don't come down for months and spread globally; the effect is insignificant and diffuse. For ground level bursts the rapidly rising fireball sucks up sand and gravel, which partially melts and is infused with fission products in the fire ball. It is too heavy and will soon fall out of the mushroom cloud when it stops rising or it happens to get to the outside edge of the toroidal fire ball where updraft is left.
The rule of thumb for the decay of fallout (which is a mix of many isotopes and not a simple exponential) is that the activity falls off by a factor of 10 each time the time elapsed since the peak dose rate was detected increases by a factor of 7.

The internet is the greatest of all inventions in some manners. It allows you to retrieve an answer to anything without caring where it came from. It allows you to be correct during a 15 minute window and know nothing about what you were correct about 24 hours later. It allows you to retrieve endless supplies of information without the experience of implementation, and pains of failure, making you better in whatever endeavor it was.
But do the people in charge want the little people to have it ? They can get access via satellite or whatever. If the wires going into worker homes fry, then why would they care. I would think that people getting “misinformation” hurts their cause. They can inform us via normal television channels of what we are allowed to believe. There are winners in a nuclear war. The people with the most to lose become the losers all else equal. Plenty of people will survive, especially among the elite, and those who strike first will be the most prepared and in the best position to succeed after the fallout.
Also I'm curious how we would even know who to retaliate against since it's possible the origin of an attack might not be immediately obvious. Would we just attack the entire world? Let's say a first strike does serious damage but still lots of people are left alive and many cities undamaged. Do we retaliate if that guarantees the rest of the cities will be nuked? Or do we cut our losses? Who do we retaliate against? What will the elites in power do who have their own selves to think about? If they're a survivor of a first strike why provoke a second?

It'd be enough of a blow to demonstrate their willingness to use nukes without ending the world. The USA would then have to choose whether to retaliate or not over a single nuke. If they don't respond immediately and Russia officially denies it and openly declares any nuclear retaliation will get an all launch response, it's going to be a hard sell for the survivors to be willing to escalate further. And there will be a lot of pressure to call a truce to avoid further escalation.

Cities are soft targets and easily get taken out with a 5-10 PSI overpressure. For cities you use an air burst to maximize the number of houses that get pushed over and this means that there is no localized fallout. For hardened targets like missile silos you need a near direct hit with a ~100 kT device and that means a detonation at or near ground level which maximizes local fallout (the kind that is actually dangerous). The way this works is that an airburst vaporizes the bomb components and they just don't come down for months and spread globally; the effect is insignificant and diffuse. For ground level bursts the rapidly rising fireball sucks up sand and gravel, which partially melts and is infused with fission products in the fire ball. It is too heavy and will soon fall out of the mushroom cloud when it stops rising or it happens to get to the outside edge of the toroidal fire ball where updraft is left.
The rule of thumb for the decay of fallout (which is a mix of many isotopes and not a simple exponential) is that the activity falls off by a factor of 10 each time the time elapsed since the peak dose rate was detected increases by a factor of 7.
E.g. if there's 1000 R/hr at 1 hr of gammas (deadly dose in about half an hour; unless you need desperately need to put out a fire you should be hunkering down), you have 100 R/hr at 7 hours (you really don't want to be outside at all; but a few minutes is not going to be that deadly), 10 R/hr at 2 days(short trips outside for emergencies are OK, but you want to hunker down most of the time), 1 R/hr at 2 weeks (you can be outside indefinitely, but you don't want to if you don't have to). This is not nearly as unsurvivable as it might seem. If it peaks at 1000 R/hr at 1 hr (a badly affected area, but not the worst affected areas which will be where multiple, overlapping nearby or upwind ground level strikes happened) a protection factor of 10 is enough to survive with some small theoretical risk of cancer 20-30 years later; that's still a lot less deadly than smoking. This is easily achievable without a fallout shelter, especially if you have a basement where you can get this by just sitting in the corner of the basement. 18 g/cm^2 (~4 ounces per square inch) of stuff shields about half of gammas. Some buckets of water; a sturdy table with some planting soil or bricks or something and pretty soon you've got another factor of 2-4 to multiply with whatever you got from your house and location in the house already.
3 feet of dirt all around you gives a protection factor of 1000. That's what a real fallout shelter does; just puts dirt between you and the radioactive bits of sand that fell out. But you don't need so much protection anywhere.
Long term inhabitability is not a thing. If it was a thing you'd be living in it right now as thousands of nukes were tested in the atmosphere. People would simply have to not be pussies and accept a fraction of a percent risk of dying of cancer 30 years later from eating local produce and weight this against starving right now.

Dirty bombs are simply too piss weak to do anything at all. Regular nuclear bombs are many orders of magnitude better at producing fallout than any dirty bomb. The effect is strongly dictated by altitude of burst and it is a real, serious but short term problem (days to weeks). Air bursts like tower test shots and what you'd use against a city will not generate local fallout and by the time fallout comes down it has already decayed to almost nothing.
Ground level bursts that you'd use against a missile silo in a farmer's field in the middle of bum-fuck-nowhere will generate deadly levels of fallout in an area of a few hundred square miles. This fallout is sand and grit and such that was sucked up and mixed with the fireball before being spat out near and down-wind of the blast. Fallout is not a goop or gas; it's just radioactive sand that will land somewhere and sit there emitting gammas.

Best place if you have a basement is in the corner of your basement, with a sturdy desk or something over you onto which you pile stuff like books, buckets of water, potting soil, brick pavers or whatever you have. Without a basement it is still survivable but needs more effort and understanding. Still, you're basically just building a heavy "pillow fort" with your heaviest furniture (like a bathtub full of water, or book shelves etc) that puts as much mass between you and the direction where the fallout is located (roof and ground outside the house) as is possible. Nearly everyone could survive fallout; but in reality many would die because they didn't understand the nature of fallout. They'd be taping up windows to keep out a toxic gas instead of scrambling to fill anything that can hold water and dirt and piling up their heaviest furniture.
