I think Alt-Right was originally a bunch of conservatives who rejected the loser cuck Republicans running. They wanted people who don't bend over to the media.
So the media started calling them Nazis. So real Nazis adopted the name too. The media only likes Republicans who sabotage themselves, their party, their country, and lose reliably. Everyone else is a "nazi". Nobody calls Romney, Jeb or McCain a nazi, because they are losers who want more future Democrats let into the country and apologize for any conservative opinions they have.
I don't know when it started this time, but they've used this label, and the corresponding posters of GOP Presidents with the Hitler mustache for at least 40 years. It's a standard tactic, maybe now extended further to aggressively label Trumps supporters too. There's nothing new under the political sun.
It goes back to the 1930's... the national socialists were to the 'right' of actual communists in the German political spectrum.
