The former are broken people - self mutilation is not the act of a healthy happy person but is driven by hatred and self-loathing. So they hack up bits off themselves, live a couple years in debilitating pain and misery, and eventually kill themselves fully rather than by half-measures.
The later are victims, no child can consent to molestation, torture and torment pushed by one or both parents or schools in which case the perps ought get treated like torturers and molesters always have.
