"A man has died and his wife is under critical care after the couple, both in their 60s, ingested chloroquine phosphate, an additive commonly used at aquariums to clean fish tanks. Within thirty minutes of ingestion, the couple experienced immediate effects requiring admittance to a nearby Banner Health hospital," the statement read.
Going to guess that the couple didn't exactly get an Rx from a doctor for the drugs . . . for some reason, I think they went to a shop and literally bought fish tank cleaning supplies, then ate those supplies.
Let me clarify: How in the world is putting a headline like that - something so brazenly misleading - not some equivalent to yelling “fire!” in a crowded theater?
Really, really stupid people get the Darwin Award. It's sad, but it's their own fault. Don't play doctor, don't take any chemical unless your doctor gives you an ok. You shouldn't have to be told that, but evidently some do.

Edit: I wish I could change the title :-/
