Itemized Bill, and that means itemize EVERYTHING.

Part of the reason you’re paying $1,500 for an anti-inflammatory shot when you pulled your back shoveling snow, is because you are paying the bills for 10 other people the hospitals are required to provide care for that have no intention of ever paying their bills.
