There's a Long Island nursing home doctor who substituted doxycycline (also antibiotic and anti inflammatory, but no cardiac side effects) for Zpac and also got good outcomes.

The cardiac issue is with azithromycin. There is a condition that is contraindicated.
I'm not an MD, but the condition apparently shows up on an EKG, and azithromycin makes it worse.
But not a problem for everyone who takes azithromycin. Allergic to Penicillin, I've taken azithromycin for decades, no issues.
