He was already cuffed. The arrest was going fine until they put him in the car, then he started complaining about how he couldn't breathe and got himself on the ground. That's what the police report says, their AG hasn't released the body cameras yet, we'd have probably heard by now if they were lying though.
I assume they called the ambulance at that point, then held him down so he'd not roll into the street or something. He was freaking out because he likely swallowed a bunch of the drug when he realized he was getting pulled over and after some minutes it killed him.
If you watch the video you can see him on the ground repositioning himself quite easily, his breathing wasn't cut off.
His death wasn't even tragic, 50k Americans die a year from fentanyl overdoses.
