Was the child wearing a button down shirt in the front or a tighter tshirt? My initial reaction is that someone grabbed his collar while he was running or pulled. This should absolutely be reported and reviewed by a trained professional. Also, do not ask him any questions about this directly or 'lead' him in conversation. If he presents any information log it. Otherwise, details can potentially be thrown as non-credible out if you try to investigate yourself.

Still ask “what happened today?” Or “how did you get hurt?”

These are ok questions. You don't want to ask, "Who did this to you?" or "Did someone do this to you?". If this happened at the school, they should provide an incident report. Did they provide any information? Present this information to the leadership of the daycare and provide them with whatever information the 'teachers' provided you.
