Since the beginning of the Republic, 66 judges (federal, including SCOTUS) have been impeached and removed. I didn't look up states' judges, but in my recollection I heard of several judges being impeached and removed during the past few decades in my own state.
It's a tough hurdle to overcome though. For example, most laypeople assume that Judge Emmet Sullivan meets the prima facie definition for malfeasance which could support a Congressional impeachment trial. I regret to inform that I don't believe that he has met that standard. However, the additional process that Judge Sullivan has put this country and the judiciary through will now be an excellent litmus test question for all further congressional nomination hearings for federal judges.
