If they need evidence tell them to talk to your lawyer. You just need his full information and details so that he can be served with a civil case.

I doubt a minimum wage employee would. If they do then you ask to speak to management. That you want a detailed report written out that the store employee has denied access to an American with a disability. That you want this information for a potential legal case. That you want the information for the legal department.

I doubt the employee will do it again. I doubt they will work there much longer. The ADA includes PTSD. You do not have to have gone through a traumatizing experience. if you believe that enforcement of masks negatively affect your mental health you can claim it.

Discrimination and failure to accommodate it is a huge liability.

Your advice is good, and yes, just be aware especially in states with varying laws.
