Yep.

I hate to say it, but I have to go to Walmart tomorrow for one fricking item. I don't want to go, I have to. (I live rural, but have made due for months with local farmers and ordering directly from USA producers...cause Bezos can suck it, too.) I will not be wearing a mask. If they refuse me service, not sure yet what I am going to do, but I am not putting that crap on.

"I have a medical condition that prevents me from wearing a mask." And that is all you need to say.

It is against the ADA (NOT HIPAA!) for them to ask what the condition is.

It is against the law for them to refuse service based on your medical condition.

It is against the law for them to refuse admission to their store based on your medical condition.

I don't think so. Usually there are multiple people in there with no masks. No one was in the store and no one was there later when I drove by. The gas station across the street doesn't require mask. Each situation is different. If people don't shop, they will change their policy. I know it's not the same for everywhere.
