The orders come from above. Doctors have best practices to follow. Most doctors are doing the right thing but they are licensed and need to act in a certain way. They can't do anything else or they lose their license. This was coordinated and it rolled downhill to such a degree that nobody could do anything. Everything from the treatments to the reporting to the data collection to the data analysis. This is so huge it's difficult to grasp just how big and how messed up and evil it is. We could be wrong and this could just be a perfect storm of incompetence but there are too many decisions at the highest levels in critical places that occam's razor doesn't seem to apply here.
