We used them in the Service obviously, you signed your name in front of a designated officer who acted as your poll judge or whatever. Paper ballots with signature, accompanied by positive ID, and that AFTER voter eligibility is confirmed. Yep.

Right now that is not the norm - we let idiot postal workers play with them for instance, with no accountability for each individual ballot or official accepting responsibility or ensuring custody.
