If the doctors' recently purchased the domain within the past 60 days, I want to say Tucows can refuse to transfer it until that period comes to an end.
"Fraud" is also a reason to refuse to transfer the domain according to ICANN rules:
I wonder if Tucows can claim the content on the doctors' website was fraudulent and refuse to transfer it.
not being able to transfer for a grace period is pretty standard practice. the two statuses: clientUpdateProhibited and clientTransferProhibited, are security measures to prevent fraud. i.e., they have to contact their registrar in order to make changes, i would assume not through a control panel.

Fraud in this context isn’t about content, it’s if you defrauded the registrar with a stolen identity, credit card etc
