Honestly I think most people give bicyclists wide berth because they're worried about hitting someone who randomly falls over and/or wipes out. I wasn't aware there was a "standard".

It may differ from place to place. Around here, usually when the bike lane starts, there's a sign with a picture of a car and a bike with 3ft between. Here's a picture.

Ah, gotcha. I don't know whether there's an "official" rule where I live or not. Just that everyone does it anyway.

Exactly, nobody is out to kill pedestrians. Where I get confronted by the spandex goofs is like a 30mph 2 lane and traffic is coming the opposite direction, or a 4 lane with traffic to my left. I can only move over so far and I'm at least doubling the space required by law. Yet they come up screaming about muh 3ft before they run the red light. Smdh! These are adults, on $5k bikes and $2k worth of spandex, not some 10 year old on a BMX bike wobbling down the bike lane.
