^^^ this is the whole thing. heart attack induced by drugs and getting himself in a frenzy.

In his addled state it was pretty clear he thought that if he could just stay out of the cop car he wouldn't go to jail.

All of the things that were agitating him were signs of being arrested.

Cuffs? "These hurt my wrists" = take these off so I'm not being arrested.

Car? "I'm claustrophobic" = let me out of the car so I'm not being arrested

When it finally got through the haze that he was actually going to jail and there was no way out he started having a panic attack or something and worked himself into a frenzy, which was too much for his heart in the midst of the drugs.

cuffs cut his wrist because he kept pulling his hands around to his front.

Yeah cause he was trying to get out and not be arrested, or possibly even to get stuff out of his pockets

It is really common, even in training people do this. I think it has to do with the illusion of control and the fact the held elbow and wrist to move the cuffed is also a lock and isn't comfortable. That said, I could only see a hand on the elbow, not wrist, which is a much nicer way of moving someone.
