I don't see a problem with that. If you request and absentee ballot and it is blank, anyone can fill it out and return it for a ballot. The information on the attached image insures that the ballot is filled out by the person requesting it. If the DOB or other info don't match, the request would be invalid.
That is a major difference between absentee ballots and massive vote by mail.
As long as the absentee ballots have some info already printed on them it might be fine, but I have to imagine some will be blank. I’d rather go in person though.
