Off the top of my head, perhaps reach out to the VA? I know their care can be a bit dubious, but it is a start. For the social aspect, that's all you! You seem to have a good head on your shoulders, and company goes a long way for someone's mental and physical health.

You are truly a wonderful human being and while I'm not particularly religious myself, I do believe there is a very special place post mortem for people like you.
