There was a report from Broward county that a worker walked into a closed room and found several poll workers filling out stacks of blank ballots. It got buried, nothing ever happened but I suspect that it's more widespread than that. You know it happened when Snopes does a hit piece on it:

Such BS. Those votes would then become hand counted votes. Ooops, they were trying to savemtime, they would say, by cheating.
It would take longer by far to fill out and carefully duplicate ballots than it would to hand count printed out ones.
