Look at the comorbidities in the table, it's stuff like pneumonia and respiratory distress. Those are things caused by covid, of course only a tiny fraction of covid deaths wouldn't have those things.
Also, america in general is super unhealthy. Especially old people, who are the bulk of covid deaths. I bet 75% of people over 75 have at least one comorbidity, so the vast majority of covid deaths having a comorbidity is pretty meaningless, especially without some baseline to compare it to.
