If OP in this situation made it clear that he was not wearing a mask due to his religion, or disability, then they cannot lawfully refuse him service. Nor can they inquire as to the disability.
From the quick search I just did it appears that the ADA only addresses employers, i.e. an employer cannot ask an employee or prospective employee if they are disabled.
My religion prohibits me from wearing a mask as there are biblical verses about the way to present yourself to the Lord.

There is justification in the bible for not wearing a mask.
