In this case, usually the city has a sheriff's department that handles specific duties of the sheriff (e.g. serving certain court mandates, evictions) and a police department that rolls up to the mayor. The sheriff does not roll up to the mayor, it's an independent office.
Baltimore City itself is a county level government entity, so fulfills county duties within the city. This is not to be confused with Baltimore County, which contains other towns and surrounds the city of Baltimore.

Off the top of my head, the other oddball cases are
-Jacksonville/Duval County, Florida -- the city takes up entire county, the governments function as one.
-Boston/Suffolk County, MA -- Boston takes up most of the county, and the Boston government performs county functions, but there are a couple of other towns in the county, such as Chelsea. Some counties in Massachusetts have no government at all, but still have a sheriff.)
-New York City takes up 5 counties in New York, each of the five boroughs is it's own county. The boroughs have their own governments and presidents that provide some minor functionality, although most government functions are performed by the city government.
Finally, the sheriff for the City of Baltimore has a website, https://sheriff.baltimorecity.gov/, which explains their enforcement authority on the front page:
"Deputy Sheriffs have the same Law Enforcement authority as the Police Department. Since the Sheriff is elected by the citizenry, only Deputy Sheriffs have the authority to enforce Civil Law in addition to Criminal and Traffic Law. "

In Vermont every officer has jurisdiction in the entire state.
