Yes it could be from 1) face mask with strap on back. 2) but it really could be from high flow 02 rebreather mask that also has head strap
The most revealing would be to look at his face for indentations, the strap often leaves visible marks on cheeks

Wearing mouth/nose covering for long periods of time, especially while speaking and increased exertion, brings down 02 et increases C02.
increase C02 can slow brain function, (losing train of thought, looking for words, weakness) plus shortness of breath.
I could, 100p, imagine his handlers putting Joe on high flow to bring up his 02 prior to his speaking engagements, to mitigate the C02 build up during his appearances.
There is no circumstance where Biden would need a nonrebreather and not have to be on oxygen 24/7.
That is emergent treatment, not "therapeutic" before a long speaking engagement.
He has dementia, not COPD.
Emergent would be life-saving measures. The non-rebreather mask would be used for hospitalized patients needing immediate intervention.
Therapeutic would be someone using oxygen all day or occasionally during minor exasperation.

There is a treatment for traumatic brain injuries called hyperbaric oxygen therapy; oxygen improves cognitive functions. Conversely, masks impair oxygen and cause hypoxia, which makes people stupid and Democrats.

Folks that transverse line on the occipital area could also represent the donor site scar from his many hair transplants to the frontal scalp. Or a visible portion of a scalp scar from his previous brain surgery(s)
Did he fly in for the event? That could easily be a strap from a nightshade which lots of people wear to nap on airplanes. Oxygen tubes go around the ears and down and it’s at the wrong angle to be either a different type of mask or a hat. This angle is for holding something over the eyes.
No way... the angle is up in back, down around ear, meaning it was holding some either at mouth or nose level... if it were to cover eyes it would be straight around, level from ear to ear...
