The Dems know its coming which is why they are now promoting the "Harris/Biden Administration" this week. They thought maybe he could hold it together until after the election but know now he won't make it that long, so they are planning on shifting in Harris soon.
The next question is who will Harris pick as her VP?
I was wondering the same thing. I think it's up to the Secretary of State to decide.
I recall some state where the candidate actually died shortly before the election. I think it was in AZ. They didn't have time to update the ballots so his name remained on the ballot. People still voted for him if I recall.
Happened in Missouri in 2000 too. Me Carnahan was running for Senate against John Ashcroft and died before the election. The Governor made it clear he would appoint Carnahan’s wife if Mel Won the election. Ashcroft (narrowly, I believed) lost and ended up as Bush’s AG.
For the presidency it doesn't matter, all you're doing is voting for your state electors in the Electoral College. Even if it's a state that tries to bind them to the party nominee, the party can nominate whoever.

How does a replacement work in terms of state ballot access?

I believe the DNC can simply install any replacements. Might not have the new names on the ballot, still will count as "D" vote.
Media might begin insisting that Harris announce who is on her VP list. Trump puts out his Supreme Court nominee list, so there's president.
Hillary. Then Kamala resigns and Hillary becomes president.
If Hillary goes on the ticket the voter fraud will have to go up exponentially because the last go around showed how much people hated her.
Donna Hylton.
