Mine (in PA) just asks for your signature, that the poll worker compares to a scanned one they have in the book.
When I first voted in 1993, they asked my last name. I gave it, and they asked me if I was my grandfather. I told them, "No. He died in 1981." They have updated their purging since then - now if you don't vote in the past 4 years, you get purged. Still sketchy.
My county makes you show ID and state your full name and address. I like it. I left out my middle name one year and they asked for it.
