It's a reference to a fictional dystopian novel The Handmaid's Tale written by Margaret Attwood.

In the novel women who are fertile (which has become rare) are kept as breeding slaves by powerful politicians and public officials so they can father children. The men who have infertile wives are assigned a handmaid who wears a long red cape (see handmaid's tale outfit). There is some weird ritual where the infertile wife holds the handmaid down while the husband impregnates her and then they raise the child as their own. But in the novel the wife and the handmaid team up and cuckhold the husband, because he's actually the one who's infertile.

As far as dystopian novels go, it wasn't a very good read. What I took away from it is that people want kids and if the human population ever declines and childbearing women are scarce, people will do anything to make babies.
