His back hurt because he it was seriously injured during WWII when his boat, the PT 109 got hit by a Japanese destroyer and ultimately sank. He was a war hero, saved some of his injured men, they were marooned on the Solomon Islands for days, and finally rescued, he received a purple heart and another medal for his leadership/courage . There is a book and a movie about it.
