I'd assume by default in storage the safety would always be on. You mean they acquired the gun without going through basic induction?

With AR-15s you can't turn the safety on unless you have a round in the chamber so I'm guessing they were given the rifle with a magazine inserted but without a round charged.

Then at some point dumbass probably pulled on the charging handle and made the weapon hot, but wasn't given any basic instruction and didn't flip the safety on.

Or, they thought they were a fucking Blackhawk Down special ops guy and was like "muh finger is muh safety"

Almost accurate. The hammer has to be back to engage the safety, You don't need a round chambered. You can even do it with the bolt back (open). You can't engage the safety if the hammer isn't charged (is forward).
