I wouldn't read too much into it. Trump does use a little hyperbole from time to time. He probably doesn't literally feel 20 years younger. It was more a figure of speech. Like when someone goes on a vacation and says they feel 10 years younger. They didn't really reverse age 10 years nor did they mean it that way.

The President understands that our country needs to see with our own eyes that he's ok in order to keep people from being afraid. Hospital food is a very balanced, healthful diet. Due to lack of appetite, he probably lost about 10-15 lbs. so that alone would make him feel better. Plus, when he looks in the mirror, his face looks slimmer (younger) from loosing the retained water.

My take on it is that he probably was given a boat load of fluids with electrolytes.
