I'm not saying they did it, but it wouldn't take the coordination you're imagining it would. The feed was coming from a single source and being redirected to multiple other outlets. Alter the main feed and every downstream feed is altered.
And if you think it couldn't be done, just look at how well any of the million video filters that you can download work. You can put dog ears and a nose on your face in an accurate position in real time and have the expressions match your face. Putting a fly on someone's head would be cake.

The feeds all come from whoever is hosting the debate, so if it's modified at the source, then it's fairly easy. It's definitely doable. All they had to do is photoshop some words on an image of a fly swatter, and list it on their existing online store. I could do that in five minutes flat. Afterwards is when they'd actually order them from a supplier.

The blur is likely Motion Smoothing or Image Interpretation.

On the other hand, if a fly knocks into your head so fast that you don't see it in the preceeding video frame, you're going to feel it unless that's a solid helmet of hair.

You can see a shift in the fly's landing as it settles.

It's just the framerate of the video. Think about how helicopter blades are captured differently on video.
