For the 25th Amendment to come into play, the Vice President and either (a) a majority of the cabinet or (b) a majority of some body created by an act of Congress for this purpose, have to go to the President of the Senate and the Speaker of the House and declare that the President is unfit. Start with this. There is no law of Congress establishing such a body. That means that to invoke the 25th Amendment right now, the Vice President and a majority of the cabinet would have to make that declaration. At that point, the President can dispute this claim, and after such a disputation, 2/3s of both houses of Congress would have to vote for the President's removal.
Obviously, this is not happening under a Trump administration.
