Age distribution is likely a huge factor. USA percent of population over 65 is 13.9%. India percent of population over 65 is only 5.7%. We know that the disease is primarily a major threat only to that older demographic (or severely sick people), and India simply has fewer people as a fraction of population in that category, thus fewer are at risk of death.
