A combo of some psycho girl they knew who walked around with her abortion in a plastic bag, and John reflecting back on all his mother's miscarriages, which he helped her pass when he was growing up (they were dirt poor so it didn't happen in a hospital). He saw those little bodies and knew them to be his brothers and sisters.
