One of those will likely be a psychological survey to gauge your level of depression. My doctor did this to me. I refused to fill it out.On the back side of the form are instructions for the doctor to measure the patient's depression depending on answers. According to that scale even if your score is zero it characterizes the patient as having mild depression.

You absolutely cannot trust your doctor in regards to this stuff. That's not to say they aren't trustworthy, but the insurance company wants them to provide more documentation to you and they're just doing what they are required to do. On it's own this form is harmless, but it's not going to be used on it's own. No one gives a shit about your mental wellbeing. They merely want to use your mental health against you for red flag laws. Meanwhile I've had a pain in my side for 10 years now that comes and goes that no doctor I've been to can find a cause for nor do they ever follow up on without my insistence, but they're being proactive about my mental wellbeing. It doesn't pass the smell test.
