Interesting point - I happen to read a health crisis book almost 10 yrs ago, that also touched about another periperial issue - American city people** are also so fragile and physically weak that a large segment of population (maybe over 30%) will NOT be able to evacuate themselves from city centers by walking 5 km. Someone interviewed in the book calls these people "a crisis factor of national security" on their own.
When the road system collapsed and obses people need to be moved one by one by special means, they may literally get "grounded" with medics / helps not even able to complete escorting out 1 out of 20 of those people even in 48 hours.
**Physically weak people is not limited to city people, but cities are usually the places with concentrated people and in need of evacuation.
