If I had unlimited power (not that I ever would), I'd say anyone who gets an abortion gets sterilized immediately. That takes care of the "medical" angle too, because if you really think you're gonna die having kids, you probably shouldn't be doing anything that would result in them. There should be no exceptions to this rule, otherwise people would just go to the first corrupt doctor they can convince to "write a note", so to speak. (And honestly if you have a kid stuck in a fallopian tube you're probably going to be sterile if you survive anyway. We are swimming in tech now, so we need somebody to make an honest try on seeing if there's a way to safely transplant the baby.)

Give it enough time and people would either accept the responsibility and have the kid, or remove themselves from the gene pool. After about 50ish years only the more responsible folks would be left having kids and the subject would likely die within a century.
