If OP was sent this ballot from his friend and he knew his DoB, his highschool friend also sent him the unique identifier. You cannot access these without it.

Additionally, you need to print these out to sign them and send them back. If OP doesn't know his signature, he can't sign them under his name.

5. You need to physically send the ballot back (or Fax)
