Maybe because it's because I'm a UK pede but I have never heard of 86 being used as a verb when talking about restaurant menu items, and I've only heard it used in the sense of to kill someone or to get rid of something permanently. My understanding is that its use moved over time from 'to kill' to 'to remove permanently'. Fowler's Modern Usage should have it as an entry, but thankfully they're not online so can't be changed.
