The Urethra is completely mangled (why they use a steel catheter instead of a plastic one, so it isn't accidentally cut). They excise the urethra from the penis, then take the last 4-5 inches, and line the inside of the new hole with it, so sex feels like sticking a catheter up your urethra, unpleasant to say the least.

They remove both testicles and the two 'muscular balloons' that are responsible for your erections. (Ironically there are similar but much smaller muscular balloons in a real clit so it will never feel like a real one, but I digress).

What is left of the skin from the penis is used to make the hole (with a portion of the urethra sewn on top of it.) They use laser hair removal to stop hair from growing inside the hole, but once the surgery is over, you can't fit a laser inside to redo it, so the hole often fills up with hair.

MTF surgery is the most grotesque violation of the hypocratic oath I have ever seen. It is no wonder that the unscrupulous surgeons who do it often fuck it up badly.
