ballots are anonymous in the United States. they dont have peoples names on them. there is no way to tie a ballot to the person who cast it except for absentee ballots, which have a name on the envelope. not surprisingly, the ballots and envelopes were separated in contested states contrary to law.
unless they replace the paper ballots afterwords
at the PA hearing witnesses claimed that trump votes were set aside and biden ballots were brought in. so they swapped the actual paper ballots.

i think there was a lawsuit that questioned legal proceedural failings in the election, so the filing clearly states that they are not alleging fraud in that specific suit.
just that point is in question. all of the other stuff is still good. It would mean that they were counting fake ballots instead of trump ballots, which is what the witnesses combined statements imply
people say that he was reporting the absentee ballot numbers mailed for the primaries. That seems to be true based on what ive seen.
