My guess: phone companies, and the verification systems, have had their employees turn on us. So, it's probably the human element. Because those companies have employees who can choose to override our security settings as the final step of password recovery, etc.
The answer is always stronger security. But that comes at a price. If human-overriding is not built into the system, you run the risk of losing access to the system unfixably.

Apparently now there are no road problems, because traffic is DUMMY THICC. It took me 90min to get to HEB, and the line was wrapped around the store. :shrug: I just decided to eat some canned soup until food is more available. All true. And I didn't expect it either. I didn't have any special supplies. I didn't even have a cooler to put my frozen food in and stick it outside. I just had a couple of boxes of protein bars, a couple of packs of water bottles, a few extra blankets, and a few long-sleeve wool shirts.
First day of The Troubles, I walked to the gas-station and bought popcorn, jerky, and canned coffee. Second day, the roads weren't so bad, and I drove to the store to buy a bunch of boxed shit. That was when the power went out, and I realized I didn't have any propane, charcoal, wood, a fireplace, or a barbecue.
I had two miserable days of sittin inside, reading while sun was out, and considering whether or not I should use my candles. I ended up lighting a few the last day before power came back at 1:30 in the morning, because I couldn't sleep and I was bored.
The coldest my house got down to was 27 degrees in livingroom. I probably didn't need to shutoff the water, but I had. Was a pain in the ass. And a pain getting everything started again.
I literally just now finished cleaning up all the extra mess I made melting snow for the toilets, wrapping fixtures in newspapers, etc.
It happens. Sometimes it floods. Sometimes there are twisters. Every once in a very, very long while, Texas will have a little quake. It's not reason to sell out your State to Biden, aoc, and Xi Jinping.
