I think the one constant that has remained throughout his life is that he's JUST smart enough to know that he's monumentally fucking dumb, and has always had such a chip on his shoulder about his own lack of intelligence, and thus is rabidly desperate to prove his superiority to others.
This is the root of his behaviour, and explains every other defect and act of evil that has arisen since his first realization of his own stupidity.

It's literally the one attack that cuts him to his core. Ever wonder why Trump doesn't punch harder about every thing that he's done? Because he doesn't need to, and to do so would be ineffective.
Trump identified his weakest spot. The part that Joe has tried to shield with fame, riches, job titles, international money hustling.
Joe Biden doesn't care if somebody calls him out on his pedophilia, his rape, or any of the other things he's done wrong. He doesn't care if somebody insults his family, outside of using it for political purposes. He can laugh it off, even if he knows he's being destroyed. Because they're punching at the shield. They're punching at a symbol of "Joe's Power".
When Trump calls Joe Biden out on his lack of intelligence, he's sidestepping the shielding. He's punching him right in that core part of him, that part that a young Joe Biden tried desperately to protect against. The part that ol' Shoe Leather tried to defend.
This is the most effective attack against Joe. This feeling of mental inadequacy is the one constant in his life. Notice how agitated he gets, especially with his diminishing faculties.
