A low level analyst at HHS could have access to enrollment and claims data as part of their everyday duties. They could just say they are looking at rates of dementia at skilled nursing facilities and boom, they have everything they need. No one would even know anything out of the ordinary had happened.
My guess is they had someone on the inside at HHS. Most nursing home residents are on Medicare by virtue of their age, and Medicare likely has detailed enrollment records/admit dates for all members at skilled nursing facilities. This list would not be hard to get for some low level analyst st HHS.
