I think there are many factors here. They get to sell 100% of the product they produce each year by charging different prices. They have high margins in the USA and they do high volume in the rest of the world.
There would be a cost both in lost sales and logistically, storage and such, if they charged USA prices everywhere.
Plus legal costs of fighting all the socialist lawyers in their countries. They would have to bribe a lot of foreign legislators to get equivalent deals to what they do here.
There is some uncertainty here, but if we assume that the pharma corps are going to chase the highest profits, which is logical, then we will pay less.
