They could be doing some computer vision analysis of an image, reduce every image to a simplified generated version and compare on that, so you can do simple image recognition that works most of the time...so you could run a feature detection algorithm, then simply compare the features that are found. You could use several feature search hueristics to construct multiple feature sets using a single image. Then anytime an image that is uploaded you would also have the feature detection run and resulting feature sets can be checked against dictionary of feature sets that you have banned.
