If by "concede" you mean "agree that he lost the election," then no, he didn't. But he did say that there will be an orderly transition of power (i.e., Trump will stop having the powers of the presidency, and Biden will start having them).

The words of the amendment say it can be used when, "the President is unable to discharge the powers and duties of his office." There is nothing in the 25th that limit that to health reasons.
Of course they can. The insurrection act is a federal law, the 25th amendment is the constitution. Once the 25th gets triggered Trump is no longer president, end of story.

It's the Secretaries of all the federal departments, like Alex Azar for HHS, etc. I have no idea what they would do, the 25th Amendment has never been used to remove a president.
Why would this be needed? If 3/5 of the states want to change a federal law, wouldn't they be able to do that through their people in Congress?
