This happened in CA before the election and it was 'huge' news here and in other conservative circles. Turns out they were legally disposing mid term election shit. I voted with a mail in ballot in 2016. Physically brought it to the polls though. My whole family did. We were told we would have to submit provisional ballots instead of not. We all received mail ballots. I did not expect to, but when I checked my voter registration on the DMV website it said I had previously opted to receive mail ballots. I dispute this because I have always voted in person.
This is what it looks like when the ballot is returned to sender, and rejected. They probably mailed one to your old address, and then when it was returned to sender they rejected it.
California sent ballots to everyone with a valid DL and voter registration on file.

I interview people like this for mid level IT positions all the time and she would never make it to the second interview. Lack of confidence is a huge red flag for me. If you can’t confidently explain a technical system you are in charge of administrating and supporting then you shouldn’t be responsible for those systems or have any credibility when talking about how they are maintained. She should have outlined her concerns before each interview and confidently and clearly asserted them in a way that makes the listener fully aware of actual problems. However the fact that someone like her secured employment there is telling to say the least.
